Important Information
Prescriptions:
Prescription requests are processed during normal offices hours, 8:30 a.m. to 5:00 p.m., Monday through Friday.
Prescription refills are not processed after hours.
X-Rays:
X-ray equipment is available in our office and is operated by licensed x-ray technicians. If your insurance company does not allow in office x-rays, our staff will help arrange an appointment for you at another facility.
The original x-rays generated by this office are part of your legal medical record. If you need copies of the x-rays, the request must be made in writing and copies paid for in advance. The fee for copying is paid to an outside facility and is established by that facility.
HIPAA (Privacy Rule):
The "privacy rule" protects health information that is maintained by physicians, hospitals, other health care providers and health plans.
Grablin Orthopaedics and our staff are committed to protecting your PHI (protected health information). Please contact our Office Manager about any concerns you have regarding your medical records at this facility.
Financial Policy:
As a medical care provider, our relationship is with you, not your insurance company. Not all services are a covered benefit in all contracts.
If your insurance plan requires written referral/authorization, it is your responsibility to make sure this office has received all required authorizations before you accept our services. You are responsible for payment of all services rendered.
Co-pays and unmet deductible amounts are required to be paid at each visit.
Most major insurance carriers are accepted, including Medicare.
